Creating a Form

When creating a new article choose the Insert Form Creator element from the drop down list and click the add button

Edit the element and then you will be in the Form Creator Options screen where you can set the following properties for your Form:

Header:
Heading Text: input any text you would like tappear at the top of the form before any form fields are displayed
General Text: input any text you would like tappear below the Heading Text but before any form fields are displayed

Submit Button:
Submit Button Type (required): This is used tset the look of the button that the user will click tsubmit the information they have entered intthe form. You can choose tuse either:
Standard Button – the button will look like a typical Windows style button or
Image Button – the look of the button will become an image that you select tuse
Submit Button Text (required): This is used tset the text that will appear on the Submit Button if you have chosen tuse the Standard Button
Submit Button Image (required): This is used tset the image that will be used for the button if you have chosen tuse the Image Button. Click on Search for an image topen the Image Library and select the image tuse. Click on Clear Image tremove the image that is currently being used.

Submission Response:
Response Type (required): This is used tset a message tdisplay tthe user when they click on the form‟s submit button. You can choose tuse either:
Pop up message – the message will appear tthe user in a small pop up window
Redirect article – the user will be redirected tanother article that exists on the system.
Pop up message (required): This is used tset the text that will display of the Pop up message option has been selected as the Response Type
Redirect Article (required): this is used tset the article that the user will be redirected tif the Redirect article option has been selected as the Response Type. You need tselect the Folder and then the Article tbe used

Miscellaneous:
Send submitted data tan email address: This is used tset an email address tsend submitted form data to. Each time the form data is submitted a new email containing that data will be sent tthis email address.

Click on the button tmove ontthe Form Creator: Fields screen.

Click on the button tcreate a new field for the form.

You now need tselect the Type of field tcreate and set its properties. You can select from the following:
Description – allows you tadd further descriptive text tthe form. This type of field is non interactive on the user side.

Text (single line input) – allows you tadd a single line text entry box tthe form.
Caption (required) – sets the heading of the field
Input Limit (required) – sets the maximum number of characters that a user can enter intthe field
Input Box Width – sets the width of the field
Mandatory Field – sets whether the field is tbe mandatory or not

Text (multi line input) – allows you tadd a multi line text entry box tthe form.
Caption (required) – sets the heading of the field
Width in pixels – sets the width of the field
Rows – sets how many rows the field show have
Mandatory Field – sets whether the field is tbe mandatory or not

Drop List Box – allows you tadd a drop down list box containing one or more options tthe form for the user tselect from
Caption (required) – sets the heading of the field
Input Box Width – sets the width of the field
List Options – sets the items that will appear in the field
Mandatory Field – sets whether the field is tbe mandatory or not

List Box – allows you tadd a list box containing one or more options tthe form for the user tselect from
Caption (required) – sets the heading of the field
Input Box Width – sets the width of the field
Input Box Height (required) – sets the height of the field
List Options – sets the items that will appear in the field
Multiple Selection – sets whether the multiple selection of items can be made or not
Mandatory Field – sets whether the field is tbe mandatory or not

Check Box – allows you tadd a single check box item tthe form
Caption (required) – sets the heading of the field
Mandatory Field – sets whether the field is tbe mandatory or not

Check Box List – allows you tadd a group of check box items tthe form where the user can select multiple choices
Caption (required) – sets the heading of field
List Options – sets the items that will appear in the field
Mandatory Field – sets whether the field is tbe mandatory or not

RadiButton List – allows you tadd a group of radibutton items tthe form where the user can only select one choice
Caption (required) – sets the heading of the field
List Options – sets the items that will appear in the field
Mandatory Field – sets whether the field is tbe mandatory or not

Date Box – allows you tadd a date entry box tthe form
Caption (required) – sets the heading of the field
Input Box Width – sets the width of the field
Mandatory Field – sets whether the field is tbe mandatory or not

Email Box – allows you tadd an email entry box tthe form
Caption (required) – sets the heading of the field
Input Limit (required) – sets the maximum number of characters that a user can enter in tthe field
Input Box Width – sets the width of the field
Mandatory Field – sets whether the field is tbe mandatory or not.

Click on the button tsave the field (in the case of the Description field this button will be labelled). Or you can click on the button tdiscard the field (in the case of the Description field this button will be labelled).

Repeat the above steps until all required fields have been added tthe form and then save
